What is Shockwave Therapy?
Shockwave therapy is a non-invasive treatment that involves creating a series of low-energy acoustic wave pulsations that are directly applied to an injury through a person’s skin via a gel medium. The concept and technology originally evolved from the discovery that focused sound waves were capable of breaking down kidneys and gallstones. Generated shockwaves have proven successful in a number of scientific studies for the treatment of chronic conditions. Shockwave therapy is its treatment for a lingering injury, or pain resulting from illness. You don’t need painkillers with it - the purpose of the therapy is to trigger the body’s natural healing response. Many people report that their pain is reduced and mobility improves after the first treatment.
How does shockwave therapy work?
Shockwave therapy is a modality that is becoming more common in physiotherapy. Using much lower energy than in medical applications, shockwave therapy, or extracorporeal shock wave therapy (ESWT), is used in the treatment of many musculoskeletal conditions, primarily those involving connective tissues such as ligaments and tendons.
There are now two common types of Extracorporeal Shock Wave Therapy (ESWT) available
1. Radial ESWT and 2. Focused ESWT
When comparing radial and focused shock wave therapy, the main considerations are penetration depth and localization of the treatment. As you’d imagine, the greater the depth penetration of the waves, the easier it is to treat deep-lying issues in the tendons, muscles or bones. Similarly, depending on the size of the problem area, the type of therapy needed will typically change.
While radial waves are good at providing relief over a wider area and can treat low depth issues such as those on the Achilles or elbow, other conditions can require more focused treatments.

Yes, it is considered very safe. No long-term adverse effects have been reported to date. However, some patients may experience minor bruising or skin irritation, which typically resolves within a few days.
How long does each session take, and how many sessions are needed?
The duration of each session depends on the patient's condition and the number of treatment areas. Generally, the shockwave treatment itself takes about 10-15 minutes. The first session may take longer due to the initial consultation and evaluation.
In most cases, shockwave therapy is combined with other physiotherapy treatments, and a typical physiotherapy session may last around 40 minutes. On average, 3 to 7 sessions are needed, but this can vary depending on the patient's specific condition.
Is Focus Shockwave Therapy Painful?
The therapy is designed to target the injured tissues while minimizing discomfort to the skin. Patients may feel moderate pain (up to 7 out of 10) in the targeted area during treatment.
Does deeper penetration lead to better results?
Not necessarily. The shockwave must reach the affected tissues to be effective. Targeting beyond the desired depth may not address the problem area and could lead to adverse effects.
Does a longer session result in better outcomes?
Shockwave therapy's effectiveness is determined by the number of doses or charges administered, not by the session's length. The therapist adjusts the machine's intensity to ensure the targeted areas receive the appropriate dosage.
Are there contraindications for Focus Shockwave Therapy?
Yes, it is not recommended for the following conditions:
- Treatment areas near the lungs
- Haemophiliac patients or those on anticoagulant therapy
- Infected treatment areas
- Patients with joint replacements, active cancer, or issues in epiphyseal regions (due to limited studies on these conditions)
